Known in medicine as rhytidectomy, a facelift does not simply remove skin that has loosened with age — it also lifts and repositions the muscle and connective tissue beneath it. The aim is not to change your face but to move the tissue back to where it used to sit, giving a rested expression. Ageing does not happen in the skin alone. Fat pads slide downwards, bone support decreases and the skin loses elasticity. Creams and superficial treatments reach a limit because they never touch all three layers, whereas facelift surgery works directly on the supporting layer.

“Facelift” is not the name of one operation but of a family of techniques. Which one is chosen depends on how far the tissue has dropped, on age, on tissue quality and on how much recovery time you can set aside.

In recent years the deep plane facelift has become known for a more natural and longer-lasting result, because it lifts tissue from a deeper layer rather than from the skin. Even so, not every patient is a candidate for the deep plane technique; in the early stages a mini facelift can achieve the goal with far less scarring and a shorter recovery.

Treatments such as Ultherapy, radiofrequency and thread lifts improve skin tightness to a degree, but none of them surgically repositions tissue that has dropped. They are not an alternative to surgery — they are supporting steps before or after it.

• Ultherapy: focused ultrasound stimulates collagen — effective in mild laxity, insufficient in advanced sagging
• Thread lift: the effect usually lasts 12 to 18 months and gives no permanent result
• Fillers and botox: manage volume and lines, but do not lift sagging tissue

One practical test: stand in front of a mirror and lift your skin upwards with your fingers. If what you see is the result you want, what you are looking for is most likely facelift surgery.

The face and the neck do not age at the same speed, but they are seen in the same frame. If only the face is treated and the neck is left alone, a visible transition line can remain under the jaw. This is why a facelift and a neck lift are planned together in a single session for most patients.

In the consultation before facelift surgery, describe your expectations as concretely as you can. Saying “I want my jawline to be defined again” rather than “I want to look younger” lets the surgeon build the plan around your priorities.

1. Initial assessment. A first remote opinion based on photographs and medical history; which technique might suit you is discussed.

3. Surgery. The facelift is carried out under general anaesthesia, 3 to 6 hours depending on scope. Incisions are hidden in the hairline and the folds of the ear.
4. The first night. Usually one night of observation in hospital.
5. Check-ups. Suture removal and dressing, followed by fit-to-fly clearance.
6. Remote follow-up. Regular photographic review after you return home.

Swelling and bruising after a facelift are expected; the real result appears as the tissue settles. The timeline below is a general framework and varies from person to person.
