Voice feminisation guide: a voice that sounds like you.
Voice feminisation surgery is a procedure on the vocal folds that permanently raises the pitch of the speaking voice; in Turkish it is known as ses inceltme, literally "voice thinning". In this guide we bring together which techniques voice feminisation is performed with, who it suits, why voice therapy is an integral part of the process, the recovery timeline and how voice feminisation surgery in Türkiye is planned.
- Voice feminisation raises the fundamental frequency of the speaking voice by shortening the vibrating part of the vocal folds or increasing their tension.
- The most commonly used method is Wendler glottoplasty, performed through the mouth with no incision in the neck.
- Oestrogen does not raise a voice that has deepened; this is why voice feminisation surgery is most often considered by trans women.
- Surgery changes the pitch of the voice; intonation, resonance and speaking style adapt to the new pitch through voice therapy.
- Complete voice rest is needed in the first week; the voice takes 3–6 months to settle, and 12 months for some people.
- No voice feminisation surgery comes with a guaranteed frequency; the result depends on the structure of your larynx.
What is voice feminisation surgery?
The main factor that makes a voice sound deep or high is the number of times the vocal folds vibrate per second (the fundamental frequency). The shorter, thinner and tighter the folds, the higher the voice sounds. Voice feminisation surgery works on one or more of these three variables to raise the pitch of the speaking voice permanently. On our voice feminisation treatment page we summarised the scope of the procedure; this guide walks you through the decision step by step.
In adult men the average fundamental frequency of the speaking voice is roughly 85–155 Hz, and in women 165–255 Hz. The 150–165 Hz band in between sounds gender-ambiguous to the ear. The aim of voice feminisation surgery is to lift the voice above this ambiguous band, into the range where the person feels at home.
- Permanently raises the pitch of the speaking voice
- Reduces the distress of being misgendered on the phone and in person
- Eases the vocal fatigue caused by constantly forcing a higher voice
- Together with voice therapy, lays the groundwork for a more natural way of speaking
Who is voice feminisation surgery suitable for?
Suitability is always decided after an ear, nose and throat examination and a voice analysis. In general, voice feminisation is considered in the following situations:
- Trans women and trans feminine people seeking voice feminisation
- Women whose voice has deepened permanently for hormonal reasons (for example androgen use or hormonal disorders)
- People who struggle in their social or working life because of how deep their voice is
- People who cannot hold their desired pitch without strain despite regular voice therapy
If there are problems affecting voice quality, such as reflux, vocal fold nodules, polyps or chronic laryngitis, these need to be treated first. In smokers, healing and voice quality are affected; stopping before surgery directly improves the result.
Voice feminisation techniques
There is no single "best" method in voice feminisation surgery. The choice depends on the structure of the larynx, the intended change in pitch and how the person uses their voice. These are the most widely used approaches today:
| Technique | How it is done | Key advantage | Keep in mind |
|---|---|---|---|
| Wendler glottoplasty | Through the mouth, under a microscope, the front part of the vocal folds is stitched together; the vibrating section becomes shorter | No neck incision and no external scar; the most common method | Complete voice rest is needed in the first week |
| Laser vocal fold reduction | Part of the vocal fold tissue is reduced with a laser, making the fold thinner and tighter | Incision-free, endoscopic | Can be used together with the Wendler method or on its own; suitability depends on anatomy |
| Cricothyroid approximation (CTA) | Through a small neck incision, two cartilages are brought closer together, tightening the folds | The vocal folds themselves are not touched | The sutures can loosen over time and reduce the gain; it is used more selectively today |
| Feminisation laryngoplasty | Through the neck, the laryngeal framework is reduced and the vocal folds are shortened | In larger larynxes, affects pitch and resonance together | A more extensive operation; leaves a scar on the neck |
The tracheal shave (Adam's apple reduction) is often confused with voice feminisation. A tracheal shave changes the appearance of the neck; it does not change the pitch of the voice. The two procedures can be addressed in the same plan for suitable patients, but because the work is close to where the vocal folds attach, the surgeon decides the order and scope.
The role of voice therapy in voice feminisation
Voice feminisation surgery changes the pitch of the voice, but pitch is not the only thing that makes a voice sound "feminine" or "masculine". Resonance (where the voice rings in the mouth and throat), intonation, stress, speaking rate and the melody at the end of words also shape perception. This is why a well-planned voice feminisation process runs alongside a voice therapist.
- Before surgery: therapy shows the current capacity of the voice and whether surgery is really needed. Some people reach the result they want with therapy alone.
- After surgery: once voice rest is over, therapy teaches you to use the new pitch without strain and helps resonance adapt to it.
- In the long term: the habit of using the voice without tiring it helps preserve the result of voice feminisation for years.
Without therapy after surgery, the speaking style can stay in old habits even if the pitch rises, and the voice may sound less natural than expected. Therapy is the step that carries the gain from surgery into everyday life.
Voice feminisation and trans people
Testosterone permanently thickens and lengthens the vocal folds during puberty. Oestrogen therapy brings many changes to the face and body, but it does not reverse this structure; in other words, hormone therapy alone does not raise the voice. For this reason voice feminisation surgery is one of the most frequently asked-about procedures in a trans woman's transition, and it is often planned together with facial and body feminisation.
Our dedicated site for trans and non-binary patients Neda Transgender Surgery explains procedures such as facial feminisation surgery (FFS) and feminisation rhinoplasty in detail. The overall framework of gender-affirming surgery and the process in Türkiye are covered on our sister site Transgender Surgery by Neda as well.
There is also a need in the opposite direction. In trans men, testosterone usually deepens the voice enough; where the deepening remains insufficient, voice masculinisation surgery may be considered. We cover this topic on our FTM voice masculinisation page.
Voice feminisation surgery step by step
- Initial assessment. A first remote review of your voice recording, medical history, hormone therapy details and expectations; we discuss whether voice feminisation surgery is right for you.
- Voice analysis and examination. On arrival in Türkiye, the structure of the vocal folds is examined endoscopically and an acoustic analysis of the voice is carried out. This recording allows the post-operative change to be compared objectively. We describe the full process on our treatment process page.
- Surgery. Wendler glottoplasty is usually performed under general anaesthesia, through the mouth, in about one hour. Techniques performed through the neck may take longer.
- The first day. Most patients are discharged the same day or after one night of observation; complete voice rest starts immediately.
- Follow-up examination. At the end of voice rest the vocal folds are examined again, and the plan for healing and returning to speech is set.
- Voice therapy and remote follow-up. Therapy begins once you are back home; regular check-ups are done with voice recordings. Months 3 and 6 are especially important.
Recovery timeline after voice feminisation surgery
Recovery after voice feminisation surgery progresses as the voice gradually "opens up". In the first weeks the voice may sound hoarse, breathy or rough; it settles as swelling goes down and the tissue heals. The timeline below is a general framework for Wendler glottoplasty; the technique used and the individual rate of healing change the timing.
| Period | What happens | What to do |
|---|---|---|
| Days 0–7 | Mild sore throat, sensitivity when swallowing | Complete voice rest: avoid speaking, whispering and forceful coughing |
| Week 2 | Voice hoarse and weak, pitch starts to rise | Short, calm conversations with your surgeon's approval |
| Weeks 3–6 | Voice gradually clears, stamina increases | Start of voice therapy, gradual return to everyday speech |
| Months 2–3 | Pitch largely settles | Continued therapy, work on resonance and intonation |
| Months 6–12 | Voice approaches its final result | Final voice analysis and review |
What to watch out for after voice feminisation surgery
In the first weeks, the stitches on the vocal folds and the healing tissue determine the quality of the result. What you do in this period matters as much as the surgery itself.
- Follow the voice rest period completely; whispering strains the vocal folds too
- Do not clear your throat forcefully; if you need to cough, cough gently and in a controlled way
- Drink plenty of water; if the air is dry, use steam or a humidifier
- If reflux treatment has been recommended, do not skip it; avoid heavy meals late in the evening
- Stay away from smoking, smoky places and alcohol at least throughout recovery
- Wait for your surgeon's approval before shouting, singing or talking in noisy places
Why choose voice feminisation in Türkiye?
Voice surgery is a narrow specialism that requires subspecialty training in laryngology. In many countries few surgeons work in this field and waiting lists are long. Türkiye stands out for experienced teams in gender-affirming surgery, including voice feminisation, hospitals of international standard and a journey that can be planned from start to finish.
- Ear, nose and throat surgeons specialised in voice disorders
- Accommodation, transfers and interpreting included in one integrated process
- Istanbul, a short flight from Europe
- Where suitable, facial or body procedures can be planned on the same trip
Among our partner doctors, Prof. Dr. Kürşat Yelkenis an ear, nose and throat and head and neck surgeon who completed subspecialty training in voice disorders at the Laryngology Department of Massachusetts Eye and Ear Infirmary (MEEI), Harvard Medical School. You can meet our other doctors among our partner specialist surgeons on our website.
Neda Deniz holds the Turkish Ministry of Health International Health Tourism Authorisation Certificate (AK-1032) and acts as an intermediary agency. All surgical procedures, including voice feminisation, are performed by partner specialist doctors in the relevant hospitals.
What determines the price of voice feminisation surgery
It is not right to give a single list price for voice feminisation; the scope differs from person to person. Health services may not advertise prices under Turkish regulations, but knowing what drives the cost makes it easier to compare quotes.
- Technique: glottoplasty through the mouth, or a method performed through the neck
- Additional procedure: whether a procedure such as a tracheal shave is done within the same plan
- Pre- and post-operative voice analysis: endoscopic examination and acoustic measurements
- Voice therapy: number of sessions and the option of remote therapy
- Hospital, anaesthesia, accommodation and transfers
Which specialist will perform the surgery, are pre- and post-operative voice analyses included, is there a check-up at the end of voice rest, how does voice therapy work and how is follow-up done once you are back home? The lowest figure usually reflects the narrowest scope.
Voice feminisation risks and realistic expectations
Like any surgery, voice feminisation carries risks. Most of them are reduced significantly by careful patient selection, an experienced team and following the post-operative rules.
- Change in voice quality: a rough, breathy or hoarser voice; usually improves over months
- Reduced vocal power: the ability to shout and speak loudly may decrease
- Change in singing range: the lower notes in particular may be lost
- Less rise than expected: the pitch may rise less than intended or drop slightly over time
- Rare problems: the stitches coming apart, granuloma formation or the need for a second procedure
The achievable pitch range depends on the structure of each person's larynx and is not the same for everyone. People who use their voice professionally, especially singers and presenters, should discuss this in detail with the surgeon when making their decision. On international health standards, the WPATH Standards of Care treat voice and communication support as part of gender-affirming care.
Other procedures planned on the same trip
Voice feminisation is usually performed on its own; however, because voice rest is a quiet recovery period, some patients want to plan facial or body procedures on the same trip. Your surgeon decides which procedures can be combined and in what order.
- Face: rhinoplasty (details in our rhinoplasty guide), a facelift for age-related changes, and a jawline-defining neck lift
- Chest: breast augmentation (see our breast augmentation guide) or, depending on your needs, breast reduction
- Body contour: liposuction, hip-shaping BBL and tummy tuck
- Hair and smile: hairline-restoring hair transplant (see our hair transplant guide) and dental treatment
All of these procedures are also summarised on our plastic surgery and cosmetic surgery pages. Every option, from a post-pregnancy mommy makeover to everything else, is listed on our treatments we arrange page.
Making your decision
Voice feminisation is not decided by a number or the lowest quote; it is an operation decided by considering the structure of your larynx, how you use your voice and your long-term expectations together. The right question is not "how many Hz will my voice be" but "which technique suits me, how will therapy support me after surgery, and who will perform it".
If you are unsure, getting an assessment first does not commit you to anything. Send us your voice recording and your questions; our partner doctors will review them and we will explain the options clearly. If voice feminisation surgery is not right for you, we will tell you that openly too.
Frequently asked questions about voice feminisation
Is the result of voice feminisation surgery permanent?
The pitch change achieved with voice feminisation surgery is designed to be permanent. However, ageing, voice habits and laryngeal health can affect the voice over time, and the values measured in the first months may shift slightly as healing completes.
How long will I be unable to speak after voice feminisation surgery?
After Wendler glottoplasty, complete voice rest of about one week is usually recommended; speaking is then increased gradually. Your surgeon sets the exact period according to the technique used, and sticking to it directly affects the result.
Does voice feminisation surgery leave a scar?
Wendler glottoplasty and laser techniques are performed through the mouth, so there is no external scar. Techniques performed through the neck, such as cricothyroid approximation and feminisation laryngoplasty, leave a small scar placed in a natural neck crease.
Will hormone therapy raise my voice?
No. Oestrogen therapy does not reverse the thickening that testosterone causes in the vocal folds. That is why the options for voice feminisation are voice therapy and, where needed, voice feminisation surgery.
Would voice therapy be enough instead of surgery?
For some people it is. Voice therapy can bring clear changes in pitch, resonance and speaking style. Voice feminisation surgery usually comes into play when the desired pitch cannot be maintained without strain through therapy alone, or as an addition to therapy.
Is voice feminisation surgery the same as a tracheal shave?
No. A tracheal shave reduces the Adam's apple and changes how the neck looks; it does not change the pitch of the voice. Voice feminisation surgery works directly on the vocal folds. Where appropriate, the two procedures can be considered within the same plan.
Can I sing after voice feminisation surgery?
Most people can keep singing, but the vocal range changes and the lower notes in particular may be lost. People who use their voice professionally should discuss this in detail with the surgeon before surgery.
How many days should I stay in Türkiye for voice feminisation surgery?
For Wendler glottoplasty a programme of 7–10 days is usually recommended: examination and voice analysis, surgery and the check-up at the end of voice rest all fit into this period. Voice therapy can continue remotely once you are back home.
When will my voice reach its final result?
The pitch change starts to be heard in the first weeks and largely settles within 2–3 months. Clarity and stamina keep improving for 6–12 months; voice therapy is decisive during this period.
What does the price of voice feminisation surgery depend on?
The technique used, any additional procedure, the pre- and post-operative voice analysis, the number of voice therapy sessions, and the scope of hospital and accommodation determine the price. An exact figure can only be shared after an examination and a personalised plan.
