Double chin surgery in Turkey: fat, muscle or skin?
Double chin surgery is the umbrella term for procedures that restore a defined angle between the jaw and the neck by correcting the fat, the slack neck muscle or the loose skin that creates fullness under the chin. It is not a single technique: the same complaint is resolved in one patient by removing fat alone, in another by repairing the muscle and in a third with a neck lift. This guide to double chin surgery in Turkey explains which method addresses which problem and what recovery looks like.
- A double chin comes from three tissues: fat under the chin, a loosened platysma muscle and skin that has lost its elasticity. The double chin surgery method is chosen according to which of these dominates.
- Chin liposuction only reduces fat; it does not tighten the skin. That is why it is enough on its own for people with elastic skin and no prominent muscle bands.
- Platysmaplasty tightens the thin muscle at the front of the neck through a small incision under the chin.
- Neck liposuction takes between 30 minutes and 2 hours, and most patients go home the same day.
- After double chin surgery, a return to desk work usually takes 7–14 days and a return to exercise at least 2–3 weeks; after fat removal, the final result settles in 3–6 months.
- Ice is not applied to the neck after double chin surgery; cold can impair the blood supply to the skin.
What is double chin surgery?
Double chin surgery is the everyday name for surgical procedures that remove fat collected under the chin, tighten a slack neck muscle or remove excess skin. There is no single medical term for it; in your file the surgeon will write submental (chin) liposuction, platysmaplasty or neck lift. Which of these is written depends on the tissue that forms the double chin.
On our neck lift page we have summarised the most extensive of these operations. This guide starts one step earlier: anyone considering double chin surgery first needs to understand which problem they actually have.
- Reduces the fat pad under the chin
- Softens the vertical muscle bands at the front of the neck
- Tightens loose neck skin
- Sharpens the angle between the jawline and the neck
Double chin surgery does not change the basic appearance of your face and does not stop ageing.
What causes a double chin?
A double chin is usually a combination of three factors: fat collected under the chin, loosening of the platysma muscle that covers the front of the neck, and skin that has lost its elasticity. The American Society of Plastic Surgeons, in its neck lift patient information , lists the same four findings: a build-up of fat under the chin, loose neck skin, muscle bands and sagging along the jawline.
- Fat: can be independent of body weight; it runs in families and also appears in slim people
- Muscle: the midline edges of the platysma separate over time and show as two vertical bands
- Skin: becomes loose with age, sun exposure and significant weight loss
- Bone structure: a small or receding chin makes even a small amount of fat look like a double chin
That is why a double chin surgery plan is based on an examination, not on photographs.
Double chin surgery: which method solves which problem?
Double chin surgery options form three steps, from the least to the most extensive: fat removal alone, muscle repair, and a neck lift in which excess skin is removed. The higher the step, the longer the incision and the longer the recovery.
| Method | What it corrects | Incision | Best for |
|---|---|---|---|
| Chin liposuction | Excess fat | Small entry point under the chin | Elastic skin, no prominent muscle bands |
| Platysmaplasty | Slack muscle, vertical bands | Short incision in the crease under the chin | Younger and middle-aged patients, mild to moderate skin laxity |
| Limited-incision neck lift | Mild excess skin | Around the ears only | Limited sagging; the effect is also limited |
| Neck lift | Skin, muscle and fat together | In front of and behind the ears, often under the chin as well | Marked skin sagging |
In many double chin surgery plans, fat removal and muscle repair are done through the same incision in the same session.
Who is chin liposuction enough for?
Chin liposuction is a double chin surgery method that is enough on its own for people with elastic skin and no prominent muscle bands. The International Society of Aesthetic Plastic Surgery (ISAPS), on its neck liposuction page , describes the suitable candidate using three criteria: fat that does not respond to diet and exercise, good skin elasticity and minimal platysma banding.
No skin is removed in liposuction; after the fat has been taken out, the surgeon relies on the skin to retract on its own. If the skin is too lax to do this, removing the fat can make sagging more visible. We explain how the technique works in our liposuction guide in detail.
- The procedure takes between 30 minutes and 2 hours
- It can be done under local anaesthesia, sedation or general anaesthesia
- Most patients go home the same day
- The final result becomes clear in 3–6 months
The fat layer acts like a cover hiding the platysma edges beneath it. If someone with muscle laxity has liposuction alone, the bands may look more prominent than before.
What is platysmaplasty and when is it needed?
Platysmaplasty tightens the thin platysma muscle, which runs from the jaw to the collarbone, by stitching it together in the midline, and it is needed when the muscle contributes to the double chin. According to Cleveland Clinic's neck lift overview , this repair can be done through a single incision under the chin; no skin is removed, it is simply released from the underlying muscle and left to redrape on its own.
The same source also describes the group this approach suits best: younger and middle-aged patients with mild to moderate excess skin. No incision in front of the ear is needed.
- If two bands running down the neck are prominent
- If the angle under the chin is blunted even though there is little fat
- If deep fat lying beneath the muscle is also to be removed
Platysmaplasty is often combined with liposuction. If the skin has become noticeably loose, it is not enough on its own; at that point the double chin surgery plan widens towards a neck lift.
Double chin surgery vs neck lift: what is the difference?
The difference lies in the skin. A limited double chin surgery deals with fat and muscle and leaves the skin where it is; in a neck lift, the excess skin is lifted and removed through incisions around the ears. The classic neck lift incision starts in the hairline at the level of the sideburn, runs in front of and behind the ear, and ends in the hair-bearing scalp at the back of the neck.
- If the problem is fat: chin liposuction
- If the problem is fat and muscle: liposuction with platysmaplasty
- If the problem is mainly skin: neck lift
- If the cheeks and jawline are also sagging: face and neck together
With the last option the neck is not treated on its own; we explain why in our facelift guide in detail. For some people over middle age who come to us for double chin surgery, recommending a broader plan is not a sales tactic but an anatomical necessity.
Non-surgical double chin removal: does it work?
In mild cases yes, but the effects are limited and mostly temporary. The American Society of Plastic Surgeons states that these treatments do not replace a surgical result, although they can postpone the need for double chin surgery. The options differ according to the tissue they target.
- Deoxycholic acid injections: break down fat cells under the chin; several sessions may be needed
- Botulinum toxin: temporarily relaxes the vertical muscle bands
- Radiofrequency and focused ultrasound: aim to tighten the skin somewhat by stimulating collagen production
None of them removes excess skin. For someone with marked sagging, the time and money spent on these treatments often only delays the decision to have double chin surgery.
What happens on the day of double chin surgery?
- Initial review by photo. Front and side photographs are reviewed together with your age and weight history, and a first opinion is shared on which step is likely to be considered.
- Face-to-face examination. The surgeon assesses your skin elasticity, how the muscle behaves when you swallow and the position of your jawbone. The double chin surgery plan is finalised at this stage.
- Anaesthesia. Double chin surgery involving fat removal only can be done under local anaesthesia or sedation; for muscle repair and a neck lift, sedation or general anaesthesia is preferred.
- The procedure. Fat is removed with a thin cannula, the muscle is stitched in the midline if needed, and in a neck lift the excess skin is removed around the ears.
- Dressing and monitoring. A compression dressing is placed around the area under the chin. A thin drain may be inserted after more extensive procedures.
- Check-up and return home. Aftercare instructions are given in writing; the overall course of your trip is described on our treatment process page.
Double chin surgery recovery: what is the timeline?
Most patients return to daily life within one to two weeks of double chin surgery, although it can take months for the swelling to settle completely and for the feeling of tightness in the neck to pass. Cleveland Clinic gives 7–14 days for returning to work and at least 2–3 weeks for exercise.
| Period | What happens | What to do |
|---|---|---|
| First few days | Swelling, bruising, tightness under the chin | Head kept above heart level, neck kept straight |
| First 5–7 days | Swelling under the dressing gradually goes down | Compression dressing worn day and night |
| Weeks 1–2 | Bruising fades, movement becomes easier | Return to desk work; dressing at night only |
| Weeks 2–3 | Numbness and tingling may persist | Gradual return to sport with surgeon approval |
| Months 3–6 | Residual swelling subsides and the contour settles | Final assessment |
In the first weeks, numbness, a pulling sensation and burning in the skin are normal. After double chin surgery involving muscle repair, tightness in the neck can last a few months; the tissue gradually adapts to its new position.
Although it may be routine after other operations, ice is not applied to the neck skin after double chin surgery. The lifted skin is fed by a delicate network of blood vessels; cold can reduce this circulation and lead to skin loss. Rather than turning your neck or bending it forward, turn with your whole body, and do not lift anything heavy.
How is double chin surgery in Turkey planned?
For double chin surgery in Turkey, the length of your stay depends on the step of the procedure: patients having fat removal alone can usually fly home within a few days, while once muscle repair or a neck lift is added, the programme usually runs beyond a week. What matters is that the first dressing change and, if there is one, removal of the drain are done before you leave the country.
- Your flight date is confirmed according to the surgeon's approval at the follow-up examination
- On the plane, use the seat headrest rather than a neck pillow; a pillow presses on the incision line
- Accommodation and transfersare planned so that you can keep your head raised for the first few nights

All procedures, including double chin surgery and neck lift , are performed by our partner specialist surgeons in the relevant hospitals. When you speak to the surgeon, ask which step they are recommending and why, how they have assessed your skin elasticity, and what the method they propose cannot correct.
What determines the cost of double chin surgery?
By law, prices for healthcare services cannot be advertised. In any case, a single figure means little, as the name double chin surgery can cover anything from half an hour of fat removal to an extensive neck lift. Double chin or neck procedures carried out for cosmetic reasons are generally not funded by the NHS, so check with your insurer if you are unsure about cover.
- Step of the procedure: liposuction alone, muscle repair or skin removal
- Type of anaesthesia: local, sedation or general
- Added procedures: facelift in the same session, procedures on the tip of the chin
- Hospital stay and monitoring period
- Dressings, check-ups, accommodation and transfers
"Double chin surgery" is not a diagnosis or the name of a technique. Your quote should state clearly whether liposuction, platysmaplasty or a neck lift will be performed, which anaesthesia will be used, and whether the dressing and follow-up examinations are included.
What are the risks of double chin surgery?
Double chin surgery is carried out on a small area, but it is not risk-free; the more extensive the procedure, the longer the list of possible problems. In Turkey, procedures are carried out in line with the principles of the Turkish Society of Plastic, Reconstructive and Aesthetic Surgeons . The risks below are those listed by plastic surgery societies for the procedures that fall under double chin surgery.
- Haematoma and bleeding: sudden, one-sided swelling in the first few days is a warning sign
- Surface irregularities and asymmetry: uneven fat removal or wavy healing of the skin
- Changes in sensation: numbness is usually temporary and rarely permanent
- Nerve injury: uncommon; it can cause temporary weakness of the lower lip
- Poor blood supply to the skin and poor wound healing: more likely in smokers
- Hair loss around the incision and visible scarring: with neck lift incisions
- Infection and blood clots: as with any surgical procedure
The most common problem, however, is not medical but about expectations: when the method does not match the problem, double chin surgery falls short and a second procedure may be needed.

Making your decision
Three questions clarify the decision on double chin surgery: Is the fullness under my chin fat I can pinch, or is it loose skin? Do I have vertical bands in my neck? Am I willing to accept a scar around my ears? The answers to the first two point to the method; the third shows where your own limit lies.
It is normal if you cannot decide on your own; the examination makes the distinction. Send us your photos, let our partner surgeons review them, and we will tell you clearly which step of double chin surgery suits you. If we recommend waiting rather than surgery, we will say so too.
Frequently asked questions about double chin surgery
Does double chin surgery leave scars?
It leaves small scars whose position depends on the method. Fat removal leaves a small entry mark under the chin. With muscle repair, the incision is hidden in the crease under the chin. With a neck lift, the scar sits around the ears and along the hairline; it fades over time but does not disappear completely.
Can double chin surgery be done under local anaesthesia?
If only chin liposuction is planned, local anaesthesia or sedation is often enough. Where muscle repair and skin removal are needed, sedation or general anaesthesia is preferred; the decision is made by the surgeon and the anaesthetist.
Does losing weight make double chin surgery unnecessary?
If the double chin is due to general excess weight, losing weight makes a clear difference. With inherited fat deposits or muscle and skin laxity, however, diet and exercise do not change the result; rapid weight loss can even make loose skin worse.
Can a double chin come back after surgery?
Fat cells that have been removed do not come back, but significant weight gain enlarges the remaining cells. Ageing also continues; skin and muscle can loosen again over the years. Keeping your weight stable and protecting your skin from the sun help the result last longer.
What is the best age for double chin surgery?
There is no fixed age limit. Neck lift candidates are mostly between thirty and seventy. In younger patients fat removal is usually considered, and in later years procedures aimed at the muscle and skin; the deciding factor is what the examination shows.
How long do you wear a compression chin strap after double chin surgery?
The compression dressing is usually worn day and night for the first five to seven days, then only at night for as long as your surgeon advises. It limits swelling and helps the skin settle smoothly onto the tissue beneath.
Is neck tightness and numbness normal after double chin surgery?
Yes. Numbness, tingling and a pulling sensation are expected in the first weeks. In patients who have had muscle repair, tightness in the neck can last a few months. If the firmness does not ease over time, affects one side only or gets worse with pain, let your team know.
Can double chin surgery and a facelift be done at the same time?
Yes, it is a common combination. If the cheeks and jawline have also sagged, correcting the neck alone can leave an unnatural transition. In that case the surgeon recommends treating both areas in a single operation with a single recovery period.
Can fat-dissolving injections replace double chin surgery?
Only in limited cases: for people with a small amount of fat and firm skin, injections can be an option, but several sessions are needed and they act on fat alone. They do not correct muscle bands or sagging skin; in those cases they are no substitute for surgery.
Does a small chin affect a double chin surgery plan?
Yes. A receding chin shortens the neck angle and makes even a small amount of fat noticeable. In such cases the surgeon looks not only at the area under the chin but also at the profile balance of the face, and explains the options together.
