What is voice deepening surgery?
For a voice to sound deep, the vocal folds need to vibrate more slowly. This can be achieved by reducing the tension of the folds or by increasing their mass. Surgical techniques are based on one of these two mechanisms.
The procedure is considered for voices that do not respond to hormone therapy, or that have become thin and weak because of age and changes in the tissue.
Who might it be suitable for?
Suitability is determined after voice analysis and laryngoscopic examination. The situations most often assessed are:
- Trans men seeking voice masculinisation
- Individuals whose voice has not deepened sufficiently despite testosterone therapy
- Presbyphonia — a voice that becomes thin and weak with age
- A weak, breathy voice caused by vocal fold atrophy or insufficiency
Assessment before surgery
The way the vocal folds close is examined endoscopically, an acoustic analysis of the voice is carried out, and how the person uses their voice in daily life is assessed.
In trans men, it is usually best to wait until the effect of hormone therapy on the voice has settled; the decision about surgery becomes clear after that period.
The recovery process
Voice rest is observed in the first days. It can take weeks for the voice to reach its final state, as the swelling resolves and the tissues settle.
Voice therapy after surgery is important, as it teaches you to use the new pitch correctly and without strain.
What you should know
The pitch that can be achieved is limited by the person's laryngeal anatomy and does not give the same result in everyone. There may be some narrowing of the vocal range, and people who use their voice professionally should discuss this in detail with the doctor before deciding.
